We at Innowave are remotely manning couple of Radiology Workflow solution for Hospital and Diagnostic centers in Singapore, Middle East and India. Invariably we get request to keep assigning DICOM studies to various locations within the network on WAN or LAN.
The Assigning Rules kept changing very frequently & was tired to keep changing my DICOM Router Code. Desperately looking for a more flexible and customizable DICOM Router in the open source community.
Then came across this web page, how to build a DICOM Router using Mirth Platform. This was a blessing for my pain, caught on with it and did a complete study on Mirth Platform. I must admit, Mirth is such a useful platform for non-programmers to build quick solution. This Platform is really a asset to the Radiology community.
Since I realized Mirth’s potential, wanted to share this with the community with an example. Read below to how to build a DICOM Router in 15 minutes without any programming knowledge. Please feel free to write to me for any queries at rady@innwoave.in.
Use Case :
Request from a customer to view the images using a Clearcanvas Workstation and also have a copy of all the images pushed to workstation in a folder. The Images should be in separate folder for every patient irrespective of the study and modality.
This use case was much wired, but customers requested this for a clinical study.
Prerequisites :
- Download and Install Mirth from this location. Please download the installer not the zip version. Zip needs bit more learning to start the Mirth Service and the Administrator Manager.

- The next step is how to use the Mirth Connect, though this link is off track from our work explaining HL7 Server but give you a good start of Mirth Connect.
- Concentrate on Creating Channels and its Transformations and steps. These are some of the very important concepts of Mirth and adds great amount of flexibility. The best way to get help on Mirth Topic is to click on item “Help on this topic” on the left pane of Mirth Connect Administrator.
Actual DICOM Router in Making…
- Create a DICOM Server under Mirth that will receive images from any modality. Mirth uses DCM4Chee3 DICOM Server, so it will be able to handle all DICOM request. DICOM details for this server: AE Title is AETITLE, Port No 104 on your local PC.
- Two destinations are created one to the Clearcanvas Workstation installed locally and the second one will write to an Innowave folder in C Drive. Each Study will be created under a new folder, name will be Patient Id.
- This link has the entire channel exported to an xml file, download this channel, import in Mirth Connect Administrator.
Play around with the channel, you will be able to learn a lot of features on Mirth Connect. This platform is highly configurable, and easily deployable. The downside I could visualize is the performance, tested with CT, MR, CR, US from One image to 600 images in a study. Not sure how this will perform for large studies like peripheral Angios.
Mirth is multi-platfrom and could work across Windows, Linux and Mac environments.

When having multiple destinations in the DICOM SCP Connector Mirth will only process the destination that is listed first.
I have a source connector of a file reader, and two destinations each with a filter.
One filter will check to see what modality type it is and route to a specific SCP, the other will do the same. In the attached channel I check for type MR to send to one spot, and type CT to send to another spot. Both destinations are never processed, only the destination on top.

ShowMirth Connect Manual
Seems like fixing issue 1008 fixed this issue too. I tested the fix by creating three channels:
Channel 1: Channel with two DICOM Senders. One sending to port 105 and one sending to port 104
Channel 2: Channel with DICOM Listener listiening on port 105
Channel 3: Channel with DICOM Listener listening on port 104.
Sent one message to channel 1 and both channel 2 and channel 3 recieved one message each as ecpected
